Cervical Disc Prostheses: What is the Current Status?

Cervical myelopathy and/or radiculopathy are among the most common indications for cervical spinal surgery. Anterior cervical discectomy and fusion (ACDF) has been considered the `reference` surgical procedure for decompression and stabilization of the cervical spine. Cervical disc arthroplasty (CDA) was developed to preserve cervical range of motion (ROM), prevent symptomatic adjacent segment disease (ASD) and improve clinical outcomes. When shortterm results of cervical disc arthroplasty were compared with fusion in randomized controlled trials, the results clearly showed that it was not inferior to fusion. When the long term results were compared, more successful results were obtained in favor of cervical disc arthroplasty (CDA) in many parameters such as adjacent segment disease (ASD), ASD reoperation rates, etc. In recent studies, multilevel cervical disc arthroplasty (MCA) in multilevel cervical degenerative disc disease, hybrid surgery (HS) combining ACDF and CDA, and CDA in patients with myelopathy have been increasingly adopted.
